Sharing Your Feedback
There are a few different ways to share feedback with us. Each one serves a slightly different purpose—helping make sure your voice reaches the right place and we can respond appropriately.
After Your Visit: Patient Surveys
You may receive an invitation to complete a survey after your visit. These surveys help us understand your experience and identify opportunities to improve.
We also use this feedback to recognize team members who are providing excellent care and communication. Summaries of patient feedback, including ratings and comments, are shared on our provider profiles to help others make informed decisions about their care.
If you receive a survey, we hope you’ll take a few minutes to complete it—it truly makes a difference.
How it works:
- Surveys are sent by our independent partner, SullivanLuallinGroup
- Patients with a valid email address may be invited up to once every six months
- Surveys are completed online and include questions about your visit, communication, and overall experience
Our Commitment to Transparency
We believe in sharing patient feedback openly.
Survey results are published on our website through our data transparency program, managed in partnership with Press Ganey. Ratings are updated regularly, and comments are reviewed before publication to protect patient privacy and ensure they are relevant to the care experience.
About published feedback:
- Both positive and negative comments are shared
- Comments may be edited or excluded if they include private health information, inappropriate language, or content unrelated to the visit
- Providers must have a minimum number of survey responses for ratings and comments to appear

If you have a question, concern, or feedback about your care or experience, we encourage you to contact us directly. This allows us to respond quickly and support you in real time.

Confidential Reporting (ComplyLine)
ComplyLine is available for reporting concerns related to ethics, compliance, or misconduct—such as privacy concerns, inappropriate behavior, or situations that don’t feel safe or aligned with our standards.
Reports can be made confidentially or anonymously and are reviewed by our compliance team.
Use ComplyLine if:
- You have concerns about privacy, ethics, or professional conduct
- You prefer to report something confidentially or anonymously
- You are not comfortable raising the concern directly
